    Registration of ultrasound and computed tomography for guidance of laparoscopic liver surgery

    Laparoscopic Ultrasound (LUS) imaging is a standard tool used for image-guidance during laparoscopic liver resection, as it provides real-time information on the internal structure of the liver. However, LUS probes are di cult to handle and their resulting images hard to interpret. Additionally, some anatomical targets such as tumours are not always visible, making the LUS guidance less e ective. To solve this problem, registration between the LUS images and a pre-operative Computed Tomography (CT) scan using information from blood vessels has been previously proposed. By merging these two modalities, the relative position between the LUS images and the anatomy of CT is obtained and both can be used to guide the surgeon. The problem of LUS to CT registration is specially challenging, as besides being a multi-modal registration, the eld of view of LUS is signi cantly smaller than that of CT. Therefore, this problem becomes poorly constrained and typically an accurate initialisation is needed. Also, the liver is highly deformed during laparoscopy, complicating the problem further. So far, the methods presented in the literature are not clinically feasible as they depend on manually set correspondences between both images. In this thesis, a solution for this registration problem that may be more transferable to the clinic is proposed. Firstly, traditional registration approaches comprised of manual initialisation and optimisation of a cost function are studied. Secondly, it is demonstrated that a globally optimal registration without a manual initialisation is possible. Finally, a new globally optimal solution that does not require commonly used tracking technologies is proposed and validated. The resulting approach provides clinical value as it does not require manual interaction in the operating room or tracking devices. Furthermore, the proposed method could potentially be applied to other image-guidance problems that require registration between ultrasound and a pre-operative scan

    Mestrado em Engenharia MecânicaNesta dissertação pretende-se fazer uso de métodos matemáticos de data mining, para trazer um novo conhecimento sobre o consumo em sistemas já implementados. Através da prospeção de dados pode ser criado não só um padrão de consumos mas também uma previsão de consumos do sistema a ser monitorizado. Com esta informação é possível detetar anomalias no funcionamento do mesmo que pode levar ao seu aumento de consumo ou atuar de forma a diminuir o mesmo nos períodos em que mais se consome. Visto ser ainda um assunto pouco estudado esta é uma primeira abordagem ao problema. Neste trabalho é proposto a implementação de uma plataforma de recolha de dados, que permita o estudo desta solução. É usada uma rede de sensores sem fios de baixo consumo, de fácil implementação e grande versatilidade. Esta plataforma de recolha de dados é usada para fazer uma recolha de dados que são tratados pelos métodos descritos. No tratamento dos dados recolhidos é possível quantificar a eficácia de cada algoritmo testado, comparando-os com os dados reais. Desta forma, são melhoradas as previsões efetuadas por este tipo de métodos matemáticos. Este estudo poderá, futuramente, ser adaptado a sistemas reais melhorando assim a eficiência energética de um sistema implementado. Este melhoramento será totalmente não invasivo, isto é, sem afetar o comportamento e sem dispendiosas alterações. Será assim alcançado um aumento de eficiência energética conseguido uso racional dos equipamentos presentes, sem um decréscimo na qualidade dos mesmos.In this master thesis is intended to make use of mathematical methods of data mining, to bring new knowledge on consumption in systems already implemented. Through the mining of data can be created not only a pattern of consumption, but also a forecast of consumption of the system monitored. With this information it is possible to detect malfunctions which may lead to their increased consumption or act in order to decrease consumption during those periods of more consumption. Since it is still the beginning of this study, this is a first approach to the problem. In this paper, it is proposed to implement a data collection platform, allowing the study of this solution. It used a network of wireless sensors, lowpower, versatile and of easy implementation. This data collection platform is used to make a collection of data which are treated by the mathematical methods described. In processing of the data collected it is possible to quantify the effectiveness of each test algorithm, by comparing them with the actual data. Thus predictions made by this type of mathematical methods are improved. This study may in future be adapted to real systems thereby improving the energy efficiency of systems implemented. This improvement is totally noninvasive, ie, without affecting its behavior and without the need for costly improvements in it. It will thus be able to increase energy efficiency achieved only by the rational use of these devices without a decrease in quality

    Fan-Slicer: A Pycuda Package for Fast Reslicing of Ultrasound Shaped Planes

    Fan-Slicer (https://github.com/UCL/fan-slicer) is a Python package that enables the fast sampling (slicing) of 2D ultrasound-shaped images from a 3D volume. To increase sampling speed, CUDA kernel functions are used in conjunction with the Pycuda package. The main features include functions to generate images from both 3D surface models and 3D volumes. Additionally, the package also allows for the sampling of images from curvilinear (fan shaped planes) and linear (rectangle shaped planes) ultrasound transducers. Potential uses of Fan-slicer include the generation of large datasets of 2D images from 3D volumes and the simulation of intra-operative data among others

    Image-guidance in endoscopic pituitary surgery: an in-silico study of errors involved in tracker-based techniques

    Background: Endoscopic endonasal surgery is an established minimally invasive technique for resecting pituitary adenomas. However, understanding orientation and identifying critical neurovascular structures in this anatomically dense region can be challenging. In clinical practice, commercial navigation systems use a tracked pointer for guidance. Augmented Reality (AR) is an emerging technology used for surgical guidance. It can be tracker based or vision based, but neither is widely used in pituitary surgery. Methods: This pre-clinical study aims to assess the accuracy of tracker-based navigation systems, including those that allow for AR. Two setups were used to conduct simulations: (1) the standard pointer setup, tracked by an infrared camera; and (2) the endoscope setup that allows for AR, using reflective markers on the end of the endoscope, tracked by infrared cameras. The error sources were estimated by calculating the Euclidean distance between a point’s true location and the point’s location after passing it through the noisy system. A phantom study was then conducted to verify the in-silico simulation results and show a working example of image-based navigation errors in current methodologies. Results: The errors of the tracked pointer and tracked endoscope simulations were 1.7 and 2.5 mm respectively. The phantom study showed errors of 2.14 and 3.21 mm for the tracked pointer and tracked endoscope setups respectively. Discussion: In pituitary surgery, precise neighboring structure identification is crucial for success. However, our simulations reveal that the errors of tracked approaches were too large to meet the fine error margins required for pituitary surgery. In order to achieve the required accuracy, we would need much more accurate tracking, better calibration and improved registration techniques

    The value of Augmented Reality in surgery — A usability study on laparoscopic liver surgery

    Augmented Reality (AR) is considered to be a promising technology for the guidance of laparoscopic liver surgery. By overlaying pre-operative 3D information of the liver and internal blood vessels on the laparoscopic view, surgeons can better understand the location of critical structures. In an effort to enable AR, several authors have focused on the development of methods to obtain an accurate alignment between the laparoscopic video image and the pre-operative 3D data of the liver, without assessing the benefit that the resulting overlay can provide during surgery. In this paper, we present a study that aims to assess quantitatively and qualitatively the value of an AR overlay in laparoscopic surgery during a simulated surgical task on a phantom setup. We design a study where participants are asked to physically localise pre-operative tumours in a liver phantom using three image guidance conditions — a baseline condition without any image guidance, a condition where the 3D surfaces of the liver are aligned to the video and displayed on a black background, and a condition where video see-through AR is displayed on the laparoscopic video. Using data collected from a cohort of 24 participants which include 12 surgeons, we observe that compared to the baseline, AR decreases the median localisation error of surgeons on non-peripheral targets from 25.8 mm to 9.2 mm. Using subjective feedback, we also identify that AR introduces usability improvements in the surgical task and increases the perceived confidence of the users. Between the two tested displays, the majority of participants preferred to use the AR overlay instead of navigated view of the 3D surfaces on a separate screen. We conclude that AR has the potential to improve performance and decision making in laparoscopic surgery, and that improvements in overlay alignment accuracy and depth perception should be pursued in the future

    Deep hashing for global registration of untracked 2D laparoscopic ultrasound to CT

    PURPOSE: The registration of Laparoscopic Ultrasound (LUS) to CT can enhance the safety of laparoscopic liver surgery by providing the surgeon with awareness on the relative positioning between critical vessels and a tumour. In an effort to provide a translatable solution for this poorly constrained problem, Content-based Image Retrieval (CBIR) based on vessel information has been suggested as a method for obtaining a global coarse registration without using tracking information. However, the performance of these frameworks is limited by the use of non-generalisable handcrafted vessel features. METHODS: We propose the use of a Deep Hashing (DH) network to directly convert vessel images from both LUS and CT into fixed size hash codes. During training, these codes are learnt from a patient-specific CT scan by supplying the network with triplets of vessel images which include both a registered and a mis-registered pair. Once hash codes have been learnt, they can be used to perform registration with CBIR methods. RESULTS: We test a CBIR pipeline on 11 sequences of untracked LUS distributed across 5 clinical cases. Compared to a handcrafted feature approach, our model improves the registration success rate significantly from 48% to 61%, considering a 20 mm error as the threshold for a successful coarse registration. CONCLUSIONS: We present the first DH framework for interventional multi-modal registration tasks. The presented approach is easily generalisable to other registration problems, does not require annotated data for training, and may promote the translation of these techniques

This overall pattern is primarily associatedwith habitat loss or degradation, and human persecution for terrestrial mam-mals, and pollution, open net fishing, climate change, and prey depletion formarine mammals. Mammals play a key role in maintaining ecosystems func-tionality and resilience, and therefore information on their distribution is cru-cial to delineate and support conservation actions. MAMMALS INPORTUGAL is a publicly available data set compiling unpublishedgeoreferenced occurrence records of 92 terrestrial, volant, and marine mam-mals in mainland Portugal and archipelagos of the Azores and Madeira thatincludes 105,026 data entries between 1873 and 2021 (72% of the data occur-ring in 2000 and 2021). The methods used to collect the data were: live obser-vations/captures (43%), sign surveys (35%), camera trapping (16%),bioacoustics surveys (4%) and radiotracking, and inquiries that represent lessthan 1% of the records.     Os processos não-lineares, processos onde não se conseguem prever acon-tecimentos futuros dada a sua natureza, são de ocorrência frequente, por isso, é importante o entendimento deste tipo de procedimentos. Esta dissertação tem como principal objetivo a implementação, teste e mo-delação de metodologias não-lineares para a modelação do sistema de Três Tan-ques, partindo de uma abordagem Caixa-Preta baseada em Redes Neuronais, e de uma abordagem Caixa-Cinzenta baseada em Princípios Físico-Matemáticos. As metodologias desenvolvidas são validadas usando o sistema de Três Tanques, sistema esse, com características MIMO, onde o objetivo é obter um modelo capaz de modelar eficazmente sistemas não-lineares.The non-linear processes, processes where it is not possible to predict future events due to their nature, are of frequent occurrence, therefore it is important understanding this type of procedures. The main goal of this thesis is the implementation of methodologies for the modeling of the Three Tank system, starting from a Black Box approach, based on Neural Networks, and a Gray Box approach, based on Mathematical and / or Physical Principles. The developed approach is validated using the Three Tank system, a sys-tem with MIMO characteristics, where the objective is to obtain a model capable of effectively model non-linear systems
